Your Privacy

One of the things that’s on all of our minds these days is privacy. When requesting help from an organization such as the North Shuswap Disaster Relief Society, a certain amount of personal, private information is required to confirm that your needs are legitimate and that your alternative sources of help have been pretty much exhausted.

Our ‘Needs Assessment Form’ has this paragraph regarding your consent for personal information use:

I, ____________________________________________ authorize the North Shuswap Disaster Relief Society
to use personal information provided in this form to assist my application for disaster relief through
the Society. I understand that information I provide may be verified as part of this application
process. I also understand that the Society may be aware of other organizations who could further
assist me. At any time, I must give explicit consent to allow the society to share data with another
organization. Other than for the purposes outlined in this consent, all information contained in this
application will be considered private and confidential and best efforts will be made to keep it safe.
My consent is valid for one year only from the date of signing.

I certify that the information contained in the following application package is true to the best of my
knowledge and belief.

Signature of Applicant Date

Types of information being collected:

  • name, address, phone and email
  • personal situation and loss
  • the type of assistance needed for survival
  • short and long term needs
  • financial or other information you provide substantiating the need
  • any other information pertinent to the specific request for assistance

Personal information gathered in the application and verification processes may be used for any purpose such as, but not limited to, verifying employment, insurance coverage, ownership of the affected property, or verification of loss.  We will not share your personal information with other organizations except with your explicit consent. Other than for the purposes outlined in the application form, any information contained in the application will be considered to be private and confidential to the Society. The consent to share information in the application is valid for only one year.

We are providing support with money that has been donated to help the people and the community recover from the fire. There are limited funds and many needs. Our goal is to minimize double-dipping and provide support to the most pressing needs. The notion is that if you want support from the society, there should be no limits imposed by the application or our verification of the data. Whether it be employment, owned property, personal investments, monthly expenses, if we feel it’s needed to come to a decision, we may ask for specific information or verification.

At that point it is your choice. If there are certain types of data collected that the applicant does not wish to provide or be shared with the society, they may so indicate. If this private information prevents assistance being provided, the applicant may choose to either not receive that assistance, or change their mind about providing, or the privacy of, that specific information.

An applicant may withdraw their request for assistance at any time by emailing the society. Under those circumstances, your application for assistance will be terminated.

Retention: As long as is necessary to provide relief services to the individuals requesting relief relating to the disaster, or up to one year, whichever is longer.
